This additional piece of the clinical integrative model utilizes a risk management system that is designed to manage patients through the continuum of care by identifying risk levels and factors like complex patients to respond proactively to close gaps within the healthcare system both clinically and financially
At Scottsdale Inpatient Specialists, we treat all medical and surgical cases. SIS is focused on continuing to deliver the ultimate patient experience , engage in multidisciplinary rounding within the Hospital that encourages communication and builds a strong relationship among the hospitalists, nurses, case managers, and care coordinators.
Utilizing the SIS post-acute program that has demonstrated to reduce the length of stay at the Hospital and improved the readmission rate.
SIS physicians are highly involved with patients, the patient's caregivers, and the patient's families. SIS will continue toeffectively communicate and educate the patient, as well as any other support systems that play a role in the patient's care. Collaboration with the family, patient, and anyone involved in the care of the patient is a critical aspect to delivering quality care
SIS physicians are aware of the multiple deficits that may exist in communication associated with hospital discharge that can negatively affect continuity of care and the contribution to adverse events. Accordingly, interventions have been created and implemented by SIS physicians from these experiences to ensure that the appropriate and pertinent information reaches the PCP
